    Dr Vladimir Shishkov

    Dr Vladimir Shishkov leads the metabolic practice at Medica Kabomed — initial consultations, assessment of eating habits, body composition, metabolic markers and the factors that influence weight.

    The approach does not start with a ready-made diet, but with a medical assessment: why the body accumulates weight, why change is difficult and which factors may be sustaining the metabolic imbalance.

    The programme below is built around measurable markers, an individual nutrition plan and follow-up points through which the change is tracked over time.

    What the consultation with Dr Shishkov includes

    The consultation includes a detailed medical history, a medical examination and up to 10 laboratory markers, included in the price. The doctor determines the necessary tests according to your complaints, your medical history and the goals set.

    After the consultation you receive a nutrition plan tailored to your needs, with a menu of familiar and accessible foods. Your progress is followed at checkpoints on day 14, 30, 60 and 90.

    Between consultations you have a convenient patient app that helps you keep track of your eating, hunger, sleep and movement. In it you will find your menu for every day, and you can share photos of your food. Your habits are followed and we work on them together with you. Once a week you fill in a short questionnaire, and the charts show you how your markers change over time.

    Before every follow-up consultation you fill in a short questionnaire about the changes. This way the doctor can review your progress in advance and give more time to discussing it during the consultation.

    Kabomed Metabolic Balance

    A doctor-led approach to your weight, built around the needs of your body.

    When weight changes with difficulty, the solution is rarely “one more diet”.

    Kabomed Metabolic Balance is a medically supervised protocol that starts with the question: which factors sustain the metabolic problem in the particular person?

    The programme starts with a medical assessment and measurable markers, to judge which factors may be involved in weight gain, difficult weight loss or metabolic imbalance.

    We assess glucose metabolism, insulin resistance, hormonal balance, the inflammatory profile, oxidative stress, sleep and recovery, medication, liver metabolism and body composition.

    The aim is to prepare an individual plan that stands on data — not on a universal diet.

    The doctor then prepares an individual plan with clear steps and follow-up points — day 14, day 30 and day 90.

    The six questions we answer

    A metabolic problem rarely has only one cause. That is why the programme starts with clear questions, which we answer through examination, measurements and medical interpretation.

    01

    Is there truly excess fat tissue — or is the problem linked to fluid retention, low muscle mass or a changed body composition?

    02

    Is there evidence of insulin resistance, prediabetes or diabetes?

    03

    Is there a hormonal factor — thyroid, PCOS, menopause, low testosterone, cortisol or other deviations?

    04

    Is there an inflammatory profile, oxidative load, fatty liver, disturbed sleep or suspected sleep apnoea?

    05

    Are there medications, diseases or habits that may make the change in weight difficult?

    06

    Which nutrition, movement and recovery plan is suitable for the particular patient?

    The factors we check

    Behind difficult weight loss or weight gain there may be more than one mechanism. Each of them has its own guiding questions, markers and tests.

    Which factors are relevant for you is judged by a doctor according to the complaints, the medical history, lifestyle and the results of the initial assessment.

    • caloric excess and low physical activity;
    • insulin resistance, prediabetes or diabetes;
    • thyroid function;
    • PCOS in women;
    • perimenopause and menopause;
    • low testosterone in men;
    • chronic stress and disturbed cortisol regulation;
    • disturbed sleep or suspected sleep apnoea;
    • fatty liver and hepatic-metabolic load;
    • an inflammatory profile and oxidative load;
    • medications that may influence weight;
    • low muscle mass and a slowed basal metabolism;
    • deficiencies — vitamin D, B12, iron or others according to the case;
    • emotional eating and eating behaviour;
    • gut complaints, bloating, intolerances or disturbed gastrointestinal comfort.

    Step by step

    The programme is followed over 90 days, with pre-defined follow-up points and measurements according to the individual plan.

    1. 1

      1. Day 0 — initial diagnostics

      We start with a medical examination, a dietetic assessment, body composition analysis, functional measurements and laboratory markers at the doctor's discretion.

      This builds the baseline — the starting picture against which every subsequent change will be compared.

    2. 2

      2. Day 14 — first correction

      Weight, waist, hunger, sleep, tolerance of the regime and the first practical changes are tracked.

      On that basis the doctor may adjust the nutrition plan, movement, recovery or the complementary recommendations.

    3. 3

      3. Day 30 — objective control

      Selected measurements are repeated — body composition, weight, waist, glucose, HRV or other markers according to the plan.

      The aim is to see whether there is a measurable change, not only a subjective feeling.

    4. 4

      4. Day 60–90 — laboratory and metabolic control

      Key markers are measured again at the doctor's discretion — for example glucose, HbA1c, lipid profile, liver markers, the inflammatory profile, d-ROMs/BAP or other markers according to the case.

    5. 5

      5. After 3 months — review and next plan

      After 90 days the doctor assesses the results, tolerance and the real change in the markers.

      With a good response a maintenance programme is prepared. If the result is not sufficient, the plan is not reduced to “more willpower”; additional factors are looked for — hormonal, metabolic, inflammatory, sleep-related, medication-related or other causes that may make the change difficult.

    A metabolic phenotype.

    After the examination and the measurements we do not start with a universal eating regime. We assess which factors are involved in the particular patient and which metabolic profile emerges.

    One person may have more than one phenotype — for example insulin resistance, hepatic-metabolic load, disturbed sleep, an inflammatory profile or low muscle mass.

    The approach to each profile is part of the medical protocol. Your plan is discussed after the examination, the results and the risk assessment.

    Insulin-resistant profile
    Inflammatory-oxidative profile
    Female hormonal profile
    Male hormonal profile
    Thyroid profile
    Sleep and suspected sleep apnoea
    Hepatic-metabolic profile
    Sarcopenic profile
    Medication-influenced profile
    Emotional-eating profile

    The nutrition algorithm

    The nutrition plan is prepared individually — according to the metabolic phenotype, the test results, body composition, accompanying diseases, lifestyle and the patient's real possibilities.

    Goal

    The usual goal is a reduction of about 5–10% of body weight over 3–6 months, when this is medically appropriate and realistic.

    Rate

    A healthy reduction is usually about 0.5–1% of body weight per week, according to the individual case.

    The regime

    We do not use a universal diet. The plan is adapted to your metabolic profile, the tests, daily life, work, sleep, physical activity and eating habits.

    The food

    We work with real, accessible and familiar foods — not with abstract menus that are hard to follow in everyday life.

    Realistic expectations are part of the method. The aim is not rapid weight loss at any cost, but sustainable change, preservation of muscle mass and better metabolic control.

    The apparatus — support, not the base

    The base of the programme is the medical diagnostics, the dietetic regime, movement, the tracking of metabolic markers and the assessment of body composition.

    The technologies are not “weight-loss devices”. They are used as complementary support for recovery, autonomic regulation, oxidative balance, sleep, microcirculation and musculoskeletal comfort, when they are suitable for the particular patient.

    BMI and body composition

    BMI is used as an initial reference, but it is not sufficient on its own. That is why we consider it together with waist, weight, bioimpedance analysis, fat mass, muscle mass, visceral fat and body water.

    This way we track not only the kilograms, but the real change in body composition.

    PEMF — Verumak

    PEMF is not a “fat-melting device”. It is used as a complementary therapy to support autonomic regulation, sleep, recovery, microcirculation and musculoskeletal comfort.

    In a suitable patient an evening or recovery protocol may be applied, tracked with HRV.

    Photobiomodulation PBM

    PBM can be used as a complementary module aimed at cellular energy, local microcirculation, tissue comfort, inflammatory regulation and recovery.

    The studies are promising, but they are no basis for presenting PBM as a standalone treatment for weight loss or metabolic disease.

    Hydrogen H₂

    Molecular hydrogen can be used as complementary support in programmes aimed at oxidative balance and recovery.

    With evidence of increased oxidative load or reduced antioxidant capacity, hydrogen water, H₂ inhalation or another form of application may be included according to the individual plan.

    The effect can be tracked with d-ROMs / BAP, when these markers are included in the programme.

    Six clinical packages

    The programme is adapted to the patient's metabolic profile, results and goals.

    Which package is suitable for you — and what exactly it includes — is determined by the doctor after the first visit.

    Package 1

    Basic metabolic start

    The starting point for most patients — a medical examination, body composition assessment, basic metabolic markers and an individual plan for the first 30 days.

    Package 2

    Insulin resistance

    With evidence of sugar cravings, abdominal fat accumulation, borderline blood sugar, raised insulin or suspected prediabetes.

    Package 3

    Female hormonal profile

    With PCOS, an irregular cycle, perimenopause, menopause or other factors that may influence weight, appetite and fluid retention.

    Package 4

    Men 40+

    With abdominal fat accumulation, a decline in muscle mass, lower physical energy or suspected hormonal and metabolic imbalance.

    Package 5

    Inflammation, oxidative stress and fatigue

    With difficult weight loss combined with fatigue, musculoskeletal discomfort, an inflammatory profile or increased oxidative load.

    Package 6

    Sleep, stress and suspected sleep apnoea

    With snoring, broken sleep, morning fatigue, night hunger, high tension or evidence of impaired recovery.

    You leave with a written summary

    After the initial assessment you receive a written summary with clear next steps.

    • your metabolic profile and the factors that may make the change in weight difficult;
    • goals for 90 days — formulated with measurable markers, not only as a wish;
    • an individual nutrition plan, adapted to your results, daily life and possibilities;
    • recommendations for movement, sleep and recovery;
    • complementary device-based support — only where indicated;
    • follow-up points and markers to track;
    • when a consultation with a specialist is necessary.

    What the programme does not do

    The programme does not promise a specific number on the scale and does not present weight loss as a matter of willpower alone.

    It does not replace medication and does not change a prescribed therapy without medical judgement. If you take medication for diabetes, blood pressure, the heart, the thyroid, hormonal balance or another condition, this is taken into account when preparing the plan.

    When the results or the complaints point to a problem that requires another specialist, we discuss it openly and refer you to a suitable consultation.

    Find out why the change is difficult

    One visit with Dr Shishkov starts the programme: a medical examination, measurements and an individual plan built on your metabolic profile.

    The aim is to see which factors may be making weight loss difficult — and which steps are suitable for you.

    Do not start with yet another diet
